Collecting Insurance Information With The Self Scheduling Widget

You can require patients with insurance to provide that information while scheduling their appointments through Simplifeye.

These settings need to be configured by the Simplifeye Customer Success Team. Please email us at help@simplifeye.co or call us at 646-681-3776.

You have multiple options to select from when defining how you would like us to collect insurance information from your patients.


None

With the insurance setting set to "None", we will not ask users if the patient has insurance, and we will not collect information pertaining to insurance.

This is the default selection.


Optional

With the insurance setting set to "Optional", we will first ask if the user if the patient has insurance. The answer to this question is required.

User selects "Yes" to having insurance

If the user indicates that the patient does have insurance, they will be asked to enter their insurance information in the following screen. The fields on this screen are optional, and we will allow the user to leave them blank to proceed.

If the user indicates the patient is not the policy holder, they will be asked to fill in the policy holder's name and date of birth. These fields are dynamic and only appear if the user selects "No" to the "Patient is policy holder?" question.

User selects "No" to having insurance

The user will be allowed to proceed to the available appointments screen without being asked any other insurance questions.


Required

With the insurance setting set to "Optional", we will first ask if the user if the patient has insurance. The answer to this question is required.

User selects "Yes" to having insurance

If the user indicates that the patient does have insurance, they will be asked to enter their insurance information in the following screen. The fields on this screen are required, and we will not allow the user to leave them blank to proceed.

Viewing Insurance Information Simplifeye Collects

Any insurance information that Simplifeye collects is available to on the referral in the Simplifeye Dashboard.
